Unlimited Visits:

With our membership, there is no limit to the number of appointments you can make as long as Dr. Kaluza agrees they are medically indicated.

 

Same or Next Day Appointments:

When you have an urgent need arise, we are here for you when you need us. We will have time set aside every day to fit in our acute patients.

 

After-Hours Access:

Dr. Kaluza is available by phone 24-7 for urgent needs. Routine requests should be reserved for office hours or sent via email or voicemail after hours.

 

Direct Physician Contact:

Southlake DPC offers its patients virtual visits via your mobile device or computer. Patients can also contact us by text messaging or email. Please note, if there is an urgent concern, patients should call our office. Non-urgent messages left outside of business hours will be returned the next business day.

 

Low cost Labs & Imaging:

Dr. Kaluza’s connections in this community allow her to be able to help her patients obtain discounts on labs and imaging. We are continuing to directly contract with suppliers so as to bring our patients even better and lower prices.

 

Consultation with Specialists:

If you require consultation with a specialist, we will work closely with that specialist.

 

Annual Exam:

Each patient should schedule an annual exam that focuses on their personal health maintenance/preventive care. We will develop strategies to keep them as healthy as possible for the year ahead.

 

Comprehensive Primary Care:

Acute and chronic disease management as well as medication management and assistance developing health and wellness plans.

Care for the Whole Family:

Dr. Kaluza provides care from two year olds to adults. We can work with your whole family to develop wellness goals and then customize goals for each individual.

 

Transparency:

There are no hidden costs. The monthly membership covers your primary care needs. The only time there are additional costs would be to cover supplies and/or pathology fees on office testing and procedures. Any additional charges will be communicated prior to service being performed.
